We are currently seeking a Behavioral Health Integration Social Worker for our Ryan NENA Community Health Center located on the Lower East Side of Manhattan.
The Behavioral Health Integration Program aims to promote collaboration between primary care providers and behavioral health care in order to provide early identification and treatment of mental health concerns to a diverse population of patients in community-based health care settings. Social workers will work at the Primary Care setting with medical staff to implement universal screening tools for depression and substance abuse, provide consultation for medical staff, and provide assessment, triage, and referral of patients in need of mental health services. Social workers will provide psychosocial services including screening, outreach, risk/psychosocial assessment, crisis intervention, determination and rapid referral to follow-up, short-term counseling, and case management to a diverse patient population. The social worker works closely with primary care providers for consultation to intra-agency personnel in an integrated health care setting. He/she functions as a team member with interdisciplinary staff and works with the community agencies to assure coordinated care for his/her patients. The Behavioral Health Integration Social Worker will work with medical staff to maximize the efficacy of screening, referral, and follow-up communication between Primary Care and Mental Health providers.
