The William F. Ryan Community Health Network is a group of not-for-profit, federally qualified health centers that deliver high quality, affordable and comprehensive medical care to diverse and underserved communities. We are currently seeking a Behavioral Health Integration Social Worker for our Ryan NENA Community Health Center located on the Lower East Side of Manhattan.


Position Overview:

The Behavioral Health Integration Program aims to promote collaboration between primary care providers and behavioral health care in order to provide early identification and treatment of mental health concerns to a diverse population of patients in community-based health care settings. Social workers will work at the Primary Care setting with medical staff to implement universal screening tools for depression and substance abuse, provide consultation for medical staff, and provide assessment, triage, and referral of patients in need of mental health services.   Social workers will provide psychosocial services including screening, outreach, risk/psychosocial assessment, crisis intervention, determination and rapid referral to follow-up, short-term counseling, and case management to a diverse patient population. The social worker works closely with primary care providers for consultation to intra-agency personnel in an integrated health care setting. He/she functions as a team member with interdisciplinary staff and works with the community agencies to assure coordinated care for his/her patients. The Behavioral Health Integration Social Worker will work with medical staff to maximize the efficacy of screening, referral, and follow-up communication between Primary Care and Mental Health providers. This work is carried out in support of the mission and goals of the Ryan Network.


Essential Functions:  

  • Screening, risk assessment, psychosocial assessments, triage, and provision of appropriate care and follow-up of patients referred to the Behavioral Health Integration Program.
  • Function as the primary contact for on-site medical staff for consultation, crisis intervention, care management, and referral management.
  • Assess, determine treatment plan, provide intervention and follow-up for patients identified as at risk for depression and substance abuse through use of screening tools.
  • Using the bio-psycho-social model, provide case management services including integrated treatment planning, advocacy, and support.
  • Provide consultations to primary care practitioners providing assessments, information, and referral as needed.
  • Act as a liaison to heath care providers, community agencies, and other staff to insure patients’ access to services within the Ryan Center and the community; facilitate communication between medical Primary Care providers and Mental Health Care providers regarding issues pertaining to patient treatment.
  • Participate in community outreach and education activities to promote the program’s services. This includes workshops on selected mental health topics to patients in the Center and in the community.
  • Maintain professional competence by attendance at professional conferences, in-service seminars, and other meetings as assigned.
  • Participate in regular supervisory meetings with Program Coordinator, Department staff meetings, and other meetings as assigned. Assess the quality of care given in consultation with other Department members and in individual and peer supervision.
  • Supervise social work students as assigned.
  • Submit statistical documentation of work performance in a timely and accurate manner, and in conformity with departmental requirements. This includes having an encounter for each face-to-face service provided.
  • Document and complete all patient assessments including intakes, psycho-social assessments, treatment plans, discharge summaries, session notes, referrals, and follow-up in a timely manner in accordance with good professional practice and agency and regulatory policy.
  • Work flexible work hours depending on needs of the program, including evenings and weekends.
  • Immediately report any problems or unusual occurrences to supervisor.
  • Perform other duties and/or projects as assigned.


Minimum Experience and Skills Required:

  • Two years’ social worker experience in a health/behavioral health facility working with children and families.
  • Experience leading groups.
  • Demonstrated track record of organization, focus, and ability to work in a fast-paced clinic.
  • Able to use a computer for word processing and scheduling appointments in a calendar.
  • Able to meet the demands of a busy clinic practice in both serving patients well and documenting the work done in a timely manner.
  • Ability to demonstrate cultural competency.


Education, Licenses and/or Certifications Required:       

  • Master in Social Work
  • Licensed Clinical Social Worker (LCSW)
  • May consider LMSWs who are eligible for LCSW exam.


Preferred Qualifications:

  • Experience treating individuals with mental illness and working with diverse ethnic populations.
  • eClinicalWorks or other electronic medical records software experience.
  • Bilingual: English/Spanish.
