Overview

Position Overview:

Serve as clinical point person for care coordination in Medication Assisted Treatment (MAT) Program by screening patients for appropriateness / inclusion in the RYAN Medication Assisted Treatment (MAT) program, providing care management and coordination of the plan of care for those patients who are being treated for opiate use disorder. This work is carried out in support of the mission and goals of Ryan Network.

Responsibilities

Essential Functions:

  • Provide direct patient care (approximately 70% of the workload) within the scope of the RN license.
  • Assist in the development of clinical policies and patient flow design, with particular attention to protocols around screening and risk stratification, and provide guidance and consultation on such as necessary.
  • Participate in developing the workflow for communication with PCPs, as well as with Suboxone certified physicians.
  • In addition to substance use treatment monitoring, (through regular interaction with behavioral health, urine drug screen tracking), the MAT RN Care Manager will be expected to coordinate screening to track some chronic disease measures while working closely with the primary care team.
  • Utilize DRVS registries and EHR data to optimize the medical care of patients in treatment for substance use disorder, enhancing preventive care and the ongoing treatment of chronic issues.
  • Care coordination likely to include obtaining prior authorizations with pharmacies and managed care organizations, “triaging” patient inquiries/urgent care needs, fielding calls from partner hospitals and collaborating organizations with the goal of improved care team flow.
  • Assure that specialty consultations are appropriately timed.
  • Establish and maintain an ongoing positive working relationship with all members of the care team. Collaborate with other staff re: pertinent patient issues.
  • Ensure MAT enrolled patients are receiving and comprehending medication adherence counseling.
  • Respect and maintain patient confidentiality in all aspects for care including electronic information.
  • Respect patients in substance use treatment, and exhibit a supportive, non-judgmental attitude.
  • Exhibit consistent, high level of attention to detail in verifying own work and the work of others.
  • Respond appropriately or seek support when confronted with cultural biases or conflicts.
  • Consistently exhibit behavior and communication skills that demonstrate the William F. Ryan Community Networks commitment to high quality customer service, including quality, care and concern, with all internal and external customers.
  • Work primarily between the West 97th Street site and Ryan Chelsea Clinton but demonstrate the flexibility to occasionally work at other sites as staffing needs dictate.
  • Attend community and Center meetings, case conferences, and training sessions as required.
  • Other duties and projects as assigned.

Qualifications

Minimum Experience and Skills Required:

  • Proven skills and knowledge base necessary for clinical decision making and care delivery.
  • Knowledge of substance abuse /misuse treatment and services.
  • Experience in health education, training and evaluating outcome measures.
  • Excellent verbal, writing, and computer skills.

 Education, Licenses and/or Certifications Required:

  • Associates degree in Nursing with minimum ten years of related experience or BS in Nursing with minimum two years of related experience.
  • NYS Licensed Registered Nurse.

 Preferred Qualifications:

  • Experience in substance use disorder treatment services.
  • Case management experience.
  • Bilingual English/Spanish.
